Getting a Referral
Before scheduling your first appointment, please check with your insurance provider to see if a referral from your primary care physician is necessary, and obtain any required authorization from your insurance company.
If a referral is needed, your primary care doctor may assist with scheduling, or you can contact our clinic directly to set up your appointment once you have the referral.
Your First Visit
For a smooth first visit, please bring these items:
- Insurance card
- List of any medications you are currently taking
- Contact details, including telephone number and mailing address
- Summary of your medical history
- Any applicable co-pay
- New patient paperwork, filled out
