How to Schedule Your First Appointment

Before scheduling your initial appointment, please verify with your insurance provider if a referral from your primary care physician is required under your policy. If necessary, obtain the appropriate authorization from your insurance company.

If a referral is needed, your primary care physician may handle scheduling your appointment directly. Once you have the referral, you can also choose to schedule the appointment yourself by contacting our ENT Clinic office directly.

Preparing for Your First Visit

To ensure a smooth and efficient first visit, please bring the following items with you:

Your insurance card

A list of any medications you are currently taking

Your contact details, including telephone number and mailing address

A summary of your medical history

Any applicable co-pay
